simplify Clear() in Window and its derivatives
...
it removes argument responsible for immediate refreshing.
since after all calls that were refreshing screen there was
call to Refresh() anyway, this switch doesn't make much sense.

use pthread_t * handlers rather than pthread_t
...
pthread-win32 doesn't accept assigning zero to pthread_t type,
so we need to use pointers instead. this is more semantic anyway.
